TERMS AND POLICIES

APPOINTMENT BOOKING & CANCELLATION POLICIES

✿ Here at QD LASHES STUDIO, we understand unforeseen events happen and you may need to cancel / reschedule your appointment. We do hope you also understand for our business that we respectfully request at least 48 hours notice for all cancellations.

  • If you can not make your appointment at the date / time scheduled, please reschedule or cancel your appointment at least 48 hours ahead of time. You can reschedule / cancel your appointment online (there’s a link included with the confirmation email - make sure you check the spam/junk folder) or simply email us eyelashextensions@qdlashesstudio.com or text us 415-729-5678.
  • Late cancellations (less than 48 hours) / Missed Appointments or “No-Shows” will be charged the full amount of the original service booked. No exceptions.
  • If for any reason, the cancellation fee can not be collected from the credit card on file (ex: credit card declined, or expired...), then it will be collected before your new appointment is made and accepted. A new appointment will not be accepted unless the cancellation fee is paid in full. No exceptions.


✿ NON-REFUNDABLE DEPOSITS FOR ALL APPOINTMENTS:

  • Since the services are reserved just for you personally, a NON-REFUNDABLE DEPOSIT ($50) is required to reserve each appointment.
  • The deposit will be held & used towards the services on the day of your scheduled appointment.
  • If you cancel your appointment 48 hours prior, the deposit will be refunded (Refunds typically take up to 15 business days to process to your payment method).
  • If you fail to cancel or reschedule your appointment 48 hours prior, the deposit will be forfeited to use towards the cancellation fee (full amount of the original service booked). No refund. No exceptions. 
  • To have an appointment scheduled in the future, another deposit will be collected.

✿ LATE CANCELLING / RESCHEDULING or SAME-DAY CANCELLING / RESCHEDULING (cancel or reschedule your appointment less than 48 hours) and missed / “NO-SHOW (not showing up at your scheduled appointment) will be charged the full amount of the original service booked. No exceptions.

✿ It’s client’s responsibility to remember all appointment dates and times to avoid late arrivals, missed appointments and being charged for the cancellation fee / same-day cancellation fee / no-show fee.


✿ After 2 no shows you will not be able to book an appointment unless it is paid in full at time of booking. No exceptions.

✿ AFTER HOUR & BEFORE HOUR APPOINTMENT POLICY: UPON AVAILABILITY, appointments can be requested for BEFORE OPENING HOURS or AFTER CLOSING HOURS, or ON CLOSED BUSINESS DAY (Sundays) for an additional convenience charge of $45. 

  • Please simply email us or text us directly for more details. Deposit $50 is required for BEFORE OPENING HOURS appointments or AFTER CLOSING HOURS, or ON CLOSED BUSINESS DAY (Sundays) appointments. 

REFUND POLICIES

NO REFUNDS ON SERVICES OR FOR ALLERGIC REACTIONS (No exceptions).

If you have any concerns with the service you received, please let us know within 48 hours and we will scheduled you an “one time fix” appointment or an "one time removal" appointment to your satisfaction at no extra charge.
After 48 hours, the “one time fix” or “one time removal” is not qualified anymore, if you would like to remove the lashes or if you would like to change the lashes, please schedule a removal and a new full set (variable prices).
Refunds are not given for services or allergic reactions (No exceptions), however, we will remove the lashes free of charge (if you let us know not longer than 48 hours after your appointment).

LATE POLICIES

* ARRIVING LATE FOR APPOINTMENT

If you are running late to an appointment, we ask that you let us know as soon as possible.

If you are 10 minutes - 20 minutes late, we will try our best to make your lashes as full as possible in the time allotted. You will still be charged full price for your service.

Also, please keep in mind, the fullness of the lash extensions will NOT be guaranteed if you arrived late for your appointment.

We will always do our best to accommodate your session but the service can not be finished if you’re late more than 20 minutes.

After 20 minutes late, we will assume you will not be showing up and release the appointment. You will still be charged NO-SHOW FEE (full price) for your service.

* RUNNING BEHIND THE SCHEDULE:

If we are running behind the schedule, and you have to wait for us, we will add additional minutes to the end of your appointment.

For example: if we are late for 10 mins, we will add 10 mins to the end of the service. If you have to leave at the time scheduled, the additional minutes will be added to your next following appointment. 

REFILLS POLICIES

Please be cautious and get your refills accordingly to avoid any disappointments.


To keep your natural lashes healthy, 1-3 week Refills are required. 


There is a “3 weeks refill policy” that requires clients to have “refill“ within 1-3 week (not longer than 3 weeks from the last visit).   

Any Refill that booked after 3 weeks from the last visit will be considered as a “New Full Set”.


You will get in as many as lashes in the time frame at the refill appointment, but for maintaining a full look, you must have at least 35% of lash-extensions (each eye), Otherwise the maximum fullness is only about 55%-65% of the initial set. 

ALLERGIES / REACTIONS POLICIES

Eyelash adhesives contain cyanoacrylates


We are highly recommended and offered all clients to have a patch test at least 72 hours before a full set of extensions are carried out. 

• If you’ve experienced allergic reactions to eyelash extensions in the past, please complete the patch test at least 72 hours before booking an appointment.


• If you decided to get the service, you accepted fully responsibility for any reactions which might occur (No exceptions).


• If you experience an allergic reaction to any of the products used during a service, the service needs to be stopped and the lashed extensions need to be removed immediately. You still have to pay for the full service. 

• Refunds are not given for allergic reactions (No exceptions).

AGREEMENT TO DO BUSINESS WITH QD LASHES STUDIO

CONSUMER DISCLOSURE

From time to time, QD LASHES STUDIO (I,we, us or Company) may be required by law to provide to you certain written notices or disclosures. Described here are the terms and conditions for providing to you such notices and disclosures electronically through the electronic signing system.   

  • Getting paper copies:

At any time, you may request from QD LASHES STUDIO a paper copy of any record provided or made available electronically to you by QD LASHES STUDIO. You will have the ability to download and print documents we send to you through email.   To request paper copies of certain information from QD LASHES STUDIO, and to withdraw your prior consent to receive notices and disclosures electronically, you must send an email message to QD LASHES STUDIO at eyelashextensions@qdlashesstudio.com.   To request delivery from QD LASHES STUDIO of paper copies of the notices and disclosures previously provided by QD LASHES STUDIO to you electronically, you must send us an e-mail to eyelashextensions@qdlashesstudio.com and in the body of such request you must state your e-mail address, full name, US Postal address, and telephone number. We will bill you for any fees at that time, if any.
All notices and disclosures will be sent to you electronically unless you tell us, otherwise in accordance with the procedures described herein, we will provide electronically to you through emails all required notices, disclosures, authorizations, acknowledgements, and other documents that are required to be provided or made available to you during the course of our relationship with you.
To reduce the chance of you inadvertently not receiving any notice or disclosure, we prefer to provide all of the required notices and disclosures to you by the same method and to the same address that you have given us. Thus, you can receive all the disclosures and notices electronically or in paper format through the paper mail delivery system.   If you do not agree with this process, please let us know as described below. Please also see the paragraph immediately above that describes the consequences of your electing not to receive delivery of the notices and disclosures electronically from us.
How to contact QD LASHES STUDIO:You may contact us to let us know of your changes as to how we may contact you electronically, to request paper copies of certain information from us, and to withdraw your prior consent to receive notices and disclosures electronically as follows: 

  • To contact us by email or send messages to: eyelashextensions@qdlashesstudio.com 
  • To let us know of a change in your e-mail address where we should send notices and disclosures electronically to you, you must send an email message to us at eyelashextensions@qdlashesstudio.com and in the body of such request you must state: your previous e-mail address, your new e-mail address.  
  • We do not require any other information from you to change your email address.

To withdraw your consent with QD LASHES STUDIOTo inform us that you no longer want to receive future notices and disclosures in electronic format you may:1/ decline to sign a document, and on the subsequent page, select the check-box indicating you wish to withdraw your consent, or you may;2/ send us an e-mail to eyelashextensions@qdlashesstudio.com and in the body of such request you must state your e-mail, full name, US Postal Address, and telephone number. We do not need any other information from you to withdraw consent.
The consequences of your withdrawing consent for online documents will be that transactions may take a longer time to process.Required hardware and software.

  • These minimum requirements are subject to change. If these requirements change, you will be asked to re-accept the disclosure. Pre-release (e.g. beta) versions of operating systems and browsers are not supported.

Acknowledging your access and consent to receive materials electronicallyTo confirm to us that you can access this information electronically, which will be similar to other electronic notices and disclosures that I will provide to you, please verify that you were able to read this electronic disclosure and that you also were able to print on paper or electronically save this page for your future reference and access or that you were able to e-mail this disclosure and consent to an address where you will be able to print on paper or save it for your future reference and access. Further, if you consent to receiving notices and disclosures exclusively in electronic format on the terms and conditions described above, please let me know by clicking the ‘I agree’ button below.By filling up the forms and booking appointments with us, You confirm that:

  • You can access and read this Electronic CONSENT TO ELECTRONIC RECEIPT OF ELECTRONIC CONSUMER DISCLOSURES document; and 
  • You can print on paper the disclosure or save or send the disclosure to a place where you can print it, for future reference and access; and 
  • Until or unless you notify QD LASHES STUDIO as described above, you consent to receive from exclusively through electronic means all notices, disclosures, authorizations, acknowledgements, and other documents that are required to be provided or made available to you by QD LASHES STUDIO during the course of your relationship with us.

QD LASHES STUDIO Reserves the Rights :  

- to accept / adjust / decline any appointment requests with / without any reasons. 

- to raise any prices of any services anytime with or without notices.

- to start / pause / change / (and/or) end the “promotions“ anytime with or without notices.  

- to refuse any services to anyone for any reasons.
